Disease Management

Disease ManagementDisease Management provides one-on-one interaction for individuals with chronic health conditions that have evidence of being costly and that can be significantly improved in both cost and quality of life through early intervention, coordination among service providers and through educational programs.

The Care Coach team is comprised of Registered Nurses, Registered Dietitians, Respiratory Therapists, Exercise Physiologists, Certified Diabetes Educators, Obstetric Nurses and Licensed Mental Health Professionals. Care Coaches proactively contact patients telephonically to monitor their progress, ensure they are following their treatment plan to achieve a healthier lifestyle and prevent future problems.

Includes Maternity Management to identify high-risk pregnancies and work with expectant mothers to decrease pre-term and complicated births.
